Personally, I took it as an insult to the president. I think if the man really wanted to hurt him, he could've run forward with a weapon or used his fists.
It's a big deal for a Muslim to throw a shoe at you and call you a dog. Not that it's ever happened to me ;) Just what my Muslim friends told me when I lived overseas.
